Peer Coaching for Healthy Living

  • The Peer Coaching for Healthy Living Program series is for people with ongoing chronic conditions such as diabetes, asthma, pain, obesity, heart conditions, high blood pressure, multiple sclerosis, etc.

  • The program matches a participant and a trained volunteer health coach, and they will meet for 30 minutes, once/week for 12 weeks, over the phone.

  • Participants can have varying chronic conditions. Participants learn the skills needed in the day-to-day management of long-term illnesses and how to maintain and/or increase life’s activities. A key objective of the program is to improve participant’s self-management by increasing goalsetting, decision making, and problem solving to identify healthy and unhealthy behaviors, and practice self-care skills.

  • There is no cost to become a coach, or a participant

  • Coaches and participants both receive a textbook manual that provides valuable information about how to manage a chronic condition
