Peer Coaching for Healthy Living

The Peer Coaching for Healthy Living Program series is for people with ongoing chronic conditions such as diabetes, asthma, pain, obesity, heart conditions, high blood pressure, multiple sclerosis, etc.

The program matches a participant and a trained volunteer health coach, and they will meet for 30 minutes, once/week for 12 weeks, over the phone.

Participants can have varying chronic conditions. Participants learn the skills needed in the day-to-day management of long-term illnesses and how to maintain and/or increase life’s activities. A key objective of the program is to improve participant’s self-management by increasing goalsetting, decision making, and problem solving to identify healthy and unhealthy behaviors, and practice self-care skills.

There is no cost to become a coach, or a participant

Coaches and participants both receive a textbook manual that provides valuable information about how to manage a chronic condition
